How can a Supercharged BBL restore volume after weight loss?
Case: ~30 kg weight loss, advanced gluteal sagging · Technique: hybrid, implant + targeted fat transfer · Result: over-the-skin filler, no excision · Stable at 2 months
The essentials: what you need to know
- After losing ~30 kg, the patient presented with advanced gluteal sagging and would normally be a candidate for an excision facelift.
- Targeted fat transfer filled the deflated skin envelope and improved the contour of the lower pole, avoiding excision.
- Fat does not tighten the skin like an excision, but in borderline cases filling the envelope can prevent a scar.
- Weight loss with GLP-1 is increasing the demand for this type of buttock surgery.
- Severe sagging still requires excision: this case was borderline and was assessed individually.

Can a BBL avoid a butt lift after weight loss?
In extreme cases, yes. When the buttocks have deflated after significant weight loss but the sagging is moderate and not severe, a Supercharged BBL A hybrid procedure can fill the sagging skin envelope with implants and fat, restoring the contour and avoiding a facelift with a scar. Severe excess skin still requires excision.
Significant weight loss transforms the body, but often leaves behind excess, sagging skin, especially in the buttocks. This patient had lost nearly 30 kg and presented with advanced gluteal sagging. On paper, she was a candidate for an excisional buttock lift. This case demonstrates how, in a critical situation, we can avoid that type of scarring.
The patient: ~30 kg lost, advanced sagging
The patient had lost almost 30 kg and presented with advanced skin laxity of the buttocks, a typical indication for excision surgery. After a significant volume loss, the gluteal region usually deflates: the fat that filled and shaped the buttocks disappears, leaving an empty, flabby layer of skin.
The conventional solution is an excisional facelift, removing excess skin through an incision above the buttocks or in the gluteal fold. It works, but it leaves a scar. This patient was right on the borderline.
When is a split inevitable and when is it not?
Severe excess skin after significant weight loss requires surgical removal; there's no way around it. For many patients with substantial weight loss, an excision facelift is the only honest option and is performed when indicated.
However, there are borderline cases of moderate sagging with deswollen fat pads where filling the fat pads with volume can restore the contour enough to avoid scarring. Distinguishing between these two scenarios is the ultimate clinical judgment.
The approach: fill the deflated envelope
In this case, targeted fat transfer filled the sagging lower skin envelope, improving the contour without excision. Instead of removing skin, we restored volume.
Autologous fat was transferred specifically to the deswollen and flaccid lower pole to fill the skin fold and smooth the contour. The hybrid implant-plus-fat component allowed for the reconstruction of lost structural projection, while the fat treated the superficial fold.
This is not skin tightening like that achieved through excision; it's filling in a sagging envelope so that it sits more smoothly.
Why was the scar avoided?
Since the sagging was borderline and not severe, filling the sac restored enough contour to make excision unnecessary. The result shown in the video at two months demonstrates a filled and smooth lower pole, without an excision scar.
At two months, the swelling has subsided and the grafted fat is established: there will be no further volume loss, so the result is stable. For this patient, avoiding scarring was a significant outcome that excision surgery could not have provided.
The GLP-1 factor: why we're seeing it more and more
GLP-1 drugs such as semaglutide produce rapid and large volume losses, and a clear increase in deflated and flabby buttocks seeking contour.
We are seeing more and more patients who have lost 20–30 kg in a relatively short time, often with GLP-1 therapy, and who present with a deflated gluteal fat pad that diet and exercise cannot restore. This is transforming contouring surgery after weight loss.
Who is a candidate for this approach?
Filling the envelope is recommended for borderline sagging after weight loss, not for severe excess skin.
| It can prevent splitting | It still needs a split. |
|---|---|
| Moderate, borderline sagging | Severe excess, hanging skin |
| Over-inflated, reasonable leather quality | Very poor skin elasticity |
| The main problem is the loss of volume | The main problem is skin redundancy |
| She wants to avoid scarring if possible. | Accepts scar for permanent correction |
FAQ
Can a BBL correct sagging skin after significant weight loss?
Not through skin tightening, but in borderline cases, targeted fat transfer can fill the sagging skin and improve the contour enough to avoid excision. If the sagging is severe, surgical skin removal is still necessary.
What alternative is there to a buttock lift with skin excision?
In select, borderline cases, a hybrid buttock augmentation using implants and fat transfer can fill the deflated skin fold without scarring. This is not always possible: significant excess skin requires a facelift with excision.
Why are more and more patients seeking buttock surgery after weight loss?
The rise of GLP-1 drugs has produced rapid and large volume weight losses, often leaving a deflated and flabby buttocks that diet and exercise cannot restore.
Does fat transfer tighten sagging skin?
It doesn't tighten the skin like an excision does. In extreme cases, it can fill the sagging skin overhang with volume so that the contour improves and the skin sits smoother.
Is a Supercharged BBL suitable after weight loss with GLP-1?
This can be beneficial for patients whose buttocks have lost volume and have moderate, not severe, sagging. The implant restores the lost projection, and the fat refines the skin overlay.
How long does it take for the results to stabilize after fat transfer?
After about two months, the inflammation has subsided, the transferred fat has taken hold, and there is no further loss of volume. The result is long-lasting if the weight remains stable.
