Why do MtF patients benefit more from buttock implants than cis women?
Trans women typically have less native soft tissue volume and a different pelvic structure. Therefore, an implant is especially valuable for achieving reliable projection. In most cases, it is combined with fat grafting for a natural contour. This represents a significant difference compared to the usual indications for cisgender women.
⚡ The essentials — what you need to know
- Body contouring is one of the most impactful and least discussed parts of the male-to-female transition.
- The age at which hormone therapy is initiated is the most decisive planning factor for the surgical outcome.
- Cirumed Audit 2024–2025: 60% choose body contouring as their third surgery, 25% as their fourth, and 15% as their first. This last figure is growing rapidly.
- For more invasive procedures, venous Doppler thromboprophylaxis is part of the current standard of care.
- Buttock implants, usually combined with fat, are central to MtF buttock feminization.
MtF Body Feminization Surgery: What we teach at the Masterclass level
Body contouring is one of the most impactful and underrepresented aspects of male-to-female transition. Much of the published literature focuses on facial, tracheal, thoracic, and genital surgery. Furthermore, it often fails to include body contouring as specific to transition. However, within the trans woman community, it has become a highly sought-after service.
This article shares the core knowledge that Dr. Aslani presented as a speaker at the ISAPS Bodymasters course, the same evidence and technique that we apply at Cirumed. More information about the ISAPS Bodymasters course →
The ISAPS Bodymasters Masterclass: what was demonstrated
Dr. Aslani led a masterclass at the ISAPS Bodymasters course, held at a university hospital in Barcelona. The first day was a cadaver workshop with live anatomical preparation, attended by approximately 40 surgeons in the lab and another 200 following the video broadcast.
In that workshop, Dr. Aslani demonstrated gluteal contouring with implants and safe fat transfer using static compartment expansion at different infiltration rates. In the main program, he gave a masterclass on gluteal implants specifically in the context of body feminization.

Why buttock implants matter more in trans women patients
Trans women benefit more from buttock implants than many cis women due to their specific anatomy. The masterclass presented updated data on complication rates with buttock implants and updated recommendations for their use.
A key point: trans women often have unique anatomical characteristics—soft tissue volume, pelvic structure, and buttocks—that make implants particularly valuable. They are typically combined with fat transfer for a natural contour. This is a significant difference compared to how implants are indicated for cis women.
Cirumed Technician: el Supercharged BBL Combining a cohesive implant with fat transfer via EVL is the core technique for MtF gluteal feminization. The implant provides structural projection; the fat refines the contour and creates a natural result.

The most important planning factor: the age at which hormone therapy is started
In our experience, the age at which a patient begins hormone therapy is often crucial for the achievable outcome and the surgical strategy. This was a central message of the masterclass, drawn from our accumulated experience.
Patients who begin hormone therapy at a younger age develop markedly different physical conditions. This difference directly influences what surgery can achieve. Therefore, assessing this factor is, in our opinion, one of the most important steps in planning body feminization.
When is body contouring done in the transition?
Based on our own 2024–2025 audit data, body contouring is increasingly being performed at the beginning of transition, and sometimes even first. Most published work on transition sequencing focuses on facial, chest, and genital surgery, overlooking body contouring. We analyzed our own case data to see how patients actually prioritize it:
Source: Cirumed internal audit 2024–2025, representative of our practice.
The most striking fact is that 15% choose body contouring first. A decade ago, this was almost nonexistent. From our patient community, the reason seems to be psychological: the impact of body shape on feeling and being perceived as a woman is very high, which raises it to a higher priority for patients.
Safety: thromboprophylaxis for more invasive procedures
For more invasive feminization procedures, we consider venous Doppler-based thromboprophylaxis to be part of current recommended practice. This group of patients has a high risk of thrombosis, and the masterclass addressed how to manage this risk.
Incorporating venous Doppler ultrasound into the protocol is, in our opinion, the current standard for these cases. Furthermore, an individualized risk assessment is essential. This reflects the seriousness with which safety must be approached as procedures become more complex.

Frequently Asked Questions about MtF Body Feminization
Why do MtF patients benefit more from buttock implants than cis women?
Anatomical differences mean that many trans women have less native soft tissue volume and a different pelvic structure. Therefore, implants provide reliable projection that fat alone cannot always achieve. In our experience, implants—usually combined with fat—are especially valuable in body feminization.
Does the age at which hormone therapy is started affect body feminization surgery?
Yes, significantly. Patients who begin hormone therapy at a younger age develop different physical characteristics. This materially affects the achievable outcome and the surgical strategy. Therefore, assessing this factor is a central part of planning any feminization procedure.
Regarding the process and safety
When during the transition is body contouring usually performed?
According to our 2024–2025 audit data, around 60% of our patients have body contouring as their third surgery (after facial and chest), 25% as their fourth step, and 15% as their first procedure — a rapidly increasing number.
Why are more and more patients choosing body contouring first?
From our patient community, the psychological impact of body contour on feeling and being perceived as a woman is very high, which puts it high on the list of priorities. A decade ago this was very uncommon; today it is a clear and growing trend.
Is body feminization surgery safe? What about blood clots?
With appropriate precautions, yes. Given that these procedures can be more invasive and the patient group has a high risk of thrombosis, we consider venous Doppler-based thromboprophylaxis to be part of the current standard of care, along with individualized assessment.
Does body shape really play a part in gender transition?
Increasingly, yes. Much of what's published focuses on facial, tracheal, chest, and genital surgery, and overlooks body contouring—however, it has become highly demanded in the trans woman community, so we treat it as a genuine and specific transition priority.
Are you considering MtF body feminization?
At Cirumed, we plan each body feminization case taking into account your anatomy, the timing of hormone therapy, where it fits into your transition, and how to do it safely. Dr. Aslani She was a speaker at the ISAPS Bodymasters masterclass dedicated to this procedure.
