High-performance body contouring without abdominoplasty: a real case in a young patient
This case shows how a powerful transformation can be achieved without tummy tuck when anatomy allows it.
The strategy combined advanced 360° lipo, Skin retraction with Renuvion J-Plasma y Ultrasound-guided BBL.
The goal was not to promise a tummy tuck abdomen, but to maximize definition and retraction with realistic and safe criteria.
Many young patients want a marked waist reduction and a more athletic contour, but reject the tummy tuck because of the scar, the downtime, or their future planning.
This case demonstrates that a very solid aesthetic change can be achieved without excisional surgeryprovided that clear anatomical criteria are met and appropriate technologies are used.
- Initial challenge: square torso, defined hip dips and little waist definition.
- Abdominal situation: mild to moderate laxity, not severe.
- Plan: controlled deflation, energy retraction and strategic volume replacement.
To better understand this approach, you can review our page on liposuction and the section of Before and after.
Starting anatomy: square silhouette and pronounced hip dips
The patient presented a structurally square trunk, with several features that explained its initial outline:
- flattened lateral gluteal contour,
- hip dips deep,
- little waist indentation,
- Abdominal skin smooth with mild to moderate redundancy.
Although there was some laxity, it was not severe enough to cause the tummy tuck was mandatory.
In young patients, we avoid excisional procedures when the skin may respond well to a strategy of deflation plus contractionalways with realistic expectations.
Surgical strategy: 360° liposuction, Renuvion, and ultrasound-guided BBL
1) High-performance 360° liposuction
A circumferential liposuction of the abdomen, flanks, waist and lumbar area was performed.
The goal was to remove approximately one 50-60% of subcutaneous fat in key areas: enough to deflate abdominal volume and reshape the waist, but without destabilizing the skin envelope.
You can find more information on our page. liposuction.
2) Skin retraction with Renuvion J-Plasma
With favorable skin quality, J-Plasma can help improve abdominal retraction, redefine natural trunk lines, and reduce the risk of sagging after intense deflation.
In this patient, it was key to achieving a cleaner central definition and a more athletic appearance.
3) Ultrasound-guided fat transfer
Using carefully processed fat, the feminization of the silhouette was reinforced through corrective filling of hip dips, lateral hip expansion and improvement of gluteal projection.
This combination —a narrower waist and better-curved hips/buttocks— was the key to going from a rectangular block to a more fluid contour.
The intraoperative turning point
As deflation and contraction progressed, the change became evident.
The waist began to take shape, the abdomen flattened with a cleaner definition, and the hip dips began to integrate into a continuous lateral curve.
It's the kind of transformation that many patients don't expect to achieve without abdominoplasty.
Postoperative care: phased compression
Compression is one of the most underrated factors in high-level body contouring results.
In this case, a two-stage protocol was followed:
- Phase 1, moderate compression in the early period: From day one. It helps control inflammation, stabilize tissues, and promote adherence without compromising grafted areas.
- Phase 2, more structured compression between weeks 3 and 6: It helps to cinch the waist, improve uniformity, and enhance the final retraction.
Patient experience: what to expect in recovery
Recovery from a BBL is not usually particularly painful, but it can be uncomfortable due to postural restrictions.
We recommend:
- 4 weeks sleeping face down to protect the graft,
- Avoid sleeping on your side, especially if hip dips have been corrected.
- assume temporary inflammation related to position and postoperative period,
- Follow the phased compression exactly as indicated.
3-month result
At three months, the contour showed remarkable skin retraction, good abdominal adherence, improvement of the waist-to-hip ratio, more harmonious gluteal roundness, and stable retention.
It is a solid example of what can be achieved when a patient rejects abdominoplasty, but does meet anatomical criteria for a non-excisional approach.
Clinical note: Renuvion, or other retraction energies, is not a substitute for an abdominoplasty when severe laxity is present. If there is significant excess skin, marked diastasis recti, or pannus, the recommended approach is usually excisional. The key remains the correct case selection.
Conclusion
This case demonstrates that a young patient with mild to moderate abdominal laxity can achieve a very powerful transformation without abdominoplasty.
Combining 360° lipo, Retraction with Renuvion J-Plasma, Ultrasound-guided BBL and a protocol of structured compressionIt is possible to achieve a feminine, fluid and high-definition contour.
FAQ: Frequently Asked Questions
Can liposuction + Renuvion replace a tummy tuck?
Not for severe laxity. It can give very good results in mild-to-moderate cases with good elasticity and realistic expectations.
What type of patient is typically a "borderline candidate" for avoiding a tummy tuck?
Skin with acceptable quality, moderate redundancy, without significant diastasis and with the possibility of controlled deflation + retraction.
When is the most reliable result seen?
Starting at 8–12 weeks, when the inflammation subsides and the retraction stabilizes, the result continues to refine in subsequent months.
Why is two-stage compression so important?
Because it stabilizes tissues, promotes adherence, and improves contour uniformity, especially when there is no excisional surgery.
