Generally, we try to avoid silicone implants in buttock surgery. Using the patients own fat gives better results, better shaping and less recovery. However, there are situation where we have no other option than to resort to silicone.
In this case, the patient was explicitly asking for a very noticeable increase in projection. She was rather thin and also had previously had multiple laparotomies, so we could not take much fat from the abdomen. The optimum treatment here is as follows: Use a submuscular silicone gel implants to create projection and combine with fat grafting to the sides of the buttocks to create roundness.
This is a technically demanding surgery but gives very good results, since silicone alone will not give roundness, just projection.
