Why the starting anatomy makes it possible
Not all patients are candidates for a volume of 1000cc per side. In this case, several anatomical factors made that volume both possible and advisable:
- Sufficient subcutaneous fat in the abdomen, flanks, and lower back — the main donor areas
- Slightly straight figure — which means that the before-and-after visual contrast is very powerful even with moderate proportions
- Moderate starting gluteal volume — the skin already had the capacity to accommodate the additional volume without excessive tension
- Good skin elasticity — essential for the skin to adapt correctly to the new contour
Therefore, when these conditions occur simultaneously, the result can be especially striking. The combination of available fat, elasticity, and a straight starting point is what allows for transformations of this magnitude in a single session.
The technique: multiplane injection
In this case, a multi-plane injection technique was used—distributed at different levels of the gluteal tissue to maximize fat integration and reduce pressure at any single point. This technique allows for the safer transfer of larger volumes than single-plane injection, as it distributes the volume more evenly and promotes vascularization of the graft.
The fat transfer in this procedure was performed strictly subcutaneously—above the muscle fascia, never within or below the muscle. This is the only technique that complies with current international safety standards, according to the guidelines of the ASERF Task Force on Gluteal Fat Grafting Safety, of which Dr. Aslani is a contributing member.
